Required Landlord Forms
All landlords wishing to participate with the HCV program must complete a landlord registration form, taxpayer identification number & certification form (W9), and a direct deposit authorization form. All housing assistance payments to landlords/owners are direct deposited on the 1st of each month after the initial lease and HAP contract have been received by our office.
